PM Pediatric Care is committed to making payment as convenient as possible for our patients. Our policy is to collect payments via credit card, flexible spending card, cash, or check at the time of service. If you have insurance, our verification system will indicate whether you have an unmet deductible and determine your expected payment.

If you have any questions, please leave a detailed voicemail message for the records request department at 516-207-7960. If you are a patient/parent/legal guardian looking to obtain medical records, please click here. The records will be sent to the patient’s portal unless otherwise indicated.

This form is for parents/legal guardians who are requesting restrictions on parental rights to consent to treatment and/or access to medical records of their children. PM requires legal documentation (such as a Court Order, Restraining Order, Custody Agreement, or other legally binding order) to accompany all requests. All documentation will be reviewed prior to implementing any valid legal restrictions of those rights.
