PM Pediatric Care, together with its operating group, and all of its owned or controlled practices, subsidiaries and affiliates (“PM Pediatric Care”, “we”, “our”, “us”) is committed to meeting our obligations under California law to maintain the privacy and security of the personal information of our workforce members who are residents of the State of California.  This Notice at Collection applies only to our current and former California-resident employees (“Employees”), contractors (“Contractors”), medical staff members (“Medical Staff”), and job applicants (“Job Applicants”, and together with Employees, Medical Staff and Contractors, “Personnel”).  Our “Workforce” means Employees, Contractors, and Medical Staff.

Job Applicants

This section (Job Applicants) applies when you interact with PM Pediatric Care as a Job Applicant before you join the PM Pediatric Care Workforce, or a decision is made that you will not join our Workforce.

Categories  The categories of information collected from Job Applicants are listed below.  Some of this information is only collected if you choose to provide it, while other categories of information are required to move forward with a job application.

PM Pediatric Care does not sell or share (as defined by California law) any of the above categories of personal information.

Purpose:  The purpose of collecting personal information from Job Applicants is to evaluate your suitability to join our workforce and determine whether to make you an offer to work with PM Pediatric Care.  The purpose of retaining certain Applicant information after a workforce decision has been made, is to:  (i) comply with applicable federal, state and local laws, regulations, and rules, including those promulgated by the Equal Employment Opportunity Commission (EEOC), (ii) assist in the management and administration of our recruiting efforts, and (iii) aide our evaluation of applications you may submit to us in the future.

PM Pediatric Care retains the personal information of Workforce members for the entire duration of your status as a member of our Workforce.  PM Pediatric Care may retain certain categories of personal information after you cease to be a member of our Workforce.  Certain categories of information will be retained indefinitely if required by law, while other categories of information will be purged in accordance with PM Pediatric Care’s organizational records retention policy then in effect.

Sources of Personal Information

PM Pediatric Care collects your personal information, or information about your dependents, emergency contacts, and beneficiaries, directly from you when you provide it.
