PM Pediatric Care, together with its operating group, and all of its owned or controlled practices, subsidiaries and affiliates (“PM Pediatric Care”, “we”, “our”, “us”) is committed to meeting our obligations under California law to maintain the privacy and security of the personal information of our workforce members who are residents of the State of California. This Notice at Collection applies only to our current and former California-resident employees (“Employees”), contractors (“Contractors”), medical staff members (“Medical Staff”), and job applicants (“Job Applicants”, and together with Employees, Medical Staff and Contractors, “Personnel”). Our “Workforce” means Employees, Contractors, and Medical Staff.
Job Applicants
This section (Job Applicants) applies when you interact with PM Pediatric Care as a Job Applicant before you join the PM Pediatric Care Workforce, or a decision is made that you will not join our Workforce.
Categories The categories of information collected from Job Applicants are listed below. Some of this information is only collected if you choose to provide it, while other categories of information are required to move forward with a job application.
- Personal identifiers such as name, address, identifier, IP address, email, social security number, driver’s license, phone number, and similar identifiers.
- Physical characteristics or description.
- Education information provided on a transcript, résumé, social media profile or other sources.
- Professional, employer or employment history. This is your resume or CV, facts about your application, questionnaire responses, interviewer impressions, evaluations, our decision about your application, and your response to any offer.
- Protected classifications under California or federal law, such as race, ethnicity, national origin, ancestry, religious creed, physical or mental disability, medical conditions, marital status, gender identity or expression, age, sexual orientation, and military or veteran status. This equality and diversity information is collected when provided through voluntary self-disclosure and other means to implement diversity programs, assess working capacity, administer health and worker’s compensation programs, and comply with applicable laws.
- Inferences drawn from information you provide reflecting preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities and aptitudes.
- Sensitive information: Racial or ethnic origin and religious or philosophical beliefs.
PM Pediatric Care does not sell or share (as defined by California law) any of the above categories of personal information.
Purpose: The purpose of collecting personal information from Job Applicants is to evaluate your suitability to join our workforce and determine whether to make you an offer to work with PM Pediatric Care. The purpose of retaining certain Applicant information after a workforce decision has been made, is to: (i) comply with applicable federal, state and local laws, regulations, and rules, including those promulgated by the Equal Employment Opportunity Commission (EEOC), (ii) assist in the management and administration of our recruiting efforts, and (iii) aide our evaluation of applications you may submit to us in the future.
Retention: Certain categories of information referenced above are retained indefinitely if required by law. Other categories of information will be purged in accordance with PM Pediatric Care’s organizational records retention policy then in effect.
PM Pediatric Care collects the following categories of personal information about Personnel:

- Personal identifiers such as name, address, identifier, IP address, email, social security number, driver’s license, phone number, and similar identifiers.

- Education information provided on a transcript, résumé, social media profile or other sources as provided or made available by an applicant or employee, or that is publicly available, including any information protected by FERPA.
- Professional, employer or employment history and information.
- Financial account information such as bank account routing information and credit card information, to administer payroll functions and expense processing purposes. This information is collected without collecting security codes, access codes, or any passwords that would permit any access into your accounts.
- Medical or health information, to comply with federal, state and local anti-discrimination and disability laws; maintain records relating to leaves of absence, family related leaves; provide workplace adjustments as may be required or permitted by law; and to design and enforce workplace policies or mandates relating to health and safety, such as vaccination and/or testing policies.
- Protected classifications under California or federal law, such as race, ethnicity, national origin, ancestry, religious creed, physical or mental disability, medical conditions, marital status, gender identity or expression, age, sexual orientation, and military or veteran status. This equality and diversity information is collected when provided through voluntary self-disclosure and other means to implement diversity programs, assess working capacity, administer health and worker’s compensation programs, and comply with applicable laws.
- Commercial information, such as records of purchases of goods or services for professional development, to reimburse you for Company-related expenses, or for purchases from our employee store.
- Internet or electronic network activity, including online interactions.
- Geolocation data, including physical location and schedule of movements between offices or facilities.
- Sensory data, including audio and visual information such as photographs or videos. Such information is collected for the purposes of identifying individuals, administering security and access controls, creating identification and secure access badges, promoting our business interests such as on our website or through marketing materials, and capturing video recordings of trainings, webinars, and other uses.
- Inferences drawn from information you provide reflecting preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities and aptitudes.
- Personal information of child dependents under age 16, when voluntarily provided by Employees in connection with the enrollment and administration of benefits and other human resource purposes that involve such dependents.
- Personal information of emergency contacts, including name and contact information and relationship to an Employee, when provided for use in the event of an emergency involving the Employee.
- Personal information of beneficiaries, including name and contact information, relationship to an Employee, social security number, and other information to process benefit claims, for the purposes of administering benefits programs for beneficiaries of Employees.
Categories of Sensitive Information:
- Social security, driver’s license, state identification card, or password, number of employees, contractors, child dependents, and beneficiaries.
- Racial or ethnic origin, religious or philosophical beliefs.
- Contents of email messages when using PM Pediatric Care information systems, and contents of text messages when using PM Pediatric Care phones or mobile devices.
PM Pediatric Care does not disclose the personal information of California residents to third parties knowingly for their direct marketing purposes. PM Pediatric Care does not sell or share (as defined by California law) any of the above categories of personal information.
The general purpose of our collection of personal information from Workforce members is the management and administration of our human resources and to operate and improve our business and services to our patients and others. This includes, without limitation:
- Onboard Personnel.
- Enroll and administer benefits for Employees.
- Conduct general business activities.
- Enter into contracts with Personnel and third parties.
- Administer job duties such as setting up accounts with services providers.
- Evaluate performance for promotions, assessing suitability for tasks and responsibilities, or taking other job actions.
- Secure and safeguard the valuable assets, information and reputation of PM Pediatric Care, including to prevent fraud and to prevent harm to any of our patients or other Personnel.
- Ensure compliance with our policies and procedures.
- Provide employment confirmation, references or other information about your employment or other engagement with us.
- Other purposes set forth for any specific category in the previous section.
- Any other purposes that are permitted or required by applicable laws, regulations, ordinances, guidance or other government or agency sources.
Retention
PM Pediatric Care retains the personal information of Workforce members for the entire duration of your status as a member of our Workforce. PM Pediatric Care may retain certain categories of personal information after you cease to be a member of our Workforce. Certain categories of information will be retained indefinitely if required by law, while other categories of information will be purged in accordance with PM Pediatric Care’s organizational records retention policy then in effect.
PM Pediatric Care collects your personal information, or information about your dependents, emergency contacts, and beneficiaries, directly from you when you provide it. We may also collect your personal information when you are on our premises or when you interact with any tools, equipment, information systems, applications or other technologies provided to our workforce, such as timekeeping applications and workforce portals, as may be permitted by law and subject to any requirements of such laws. We may supplement personal information that you provide with information collected from other sources. For example, we may receive and collect information from:
- Recruiters, placement firms, referrers, and job application websites and services, including Workday.
- Prior employers, when they provide references or employment confirmation.
- Professional references you provide to us.
- Educational institutions.
- Pre-employment screening services, background check providers, Employment Eligibility Verification Services, the Department of Labor, military verification services, and consumer reporting agencies.
- Credentialing and licensing organizations.
- Publicly available sources, such as your social media profile (including LinkedIn, Twitter and Facebook).
- Third parties as necessary to provide you with benefits and ancillary services.
- Other sources as you may direct, such as your health care provider with your authorization.
We may share your information with the following parties and entities, subject to and to the extent permitted by law:
- Our Affiliates and Medical Practices.
- Service providers, including those that assist with our human resources, employee, payroll, timekeeping, benefits and other workforce management and administration; business operations; data analysis; administrative services; security services and breach forensics; and technology providers.
- Professional advisers, including legal counsel and accounting professionals.
- Insurers and/or or benefits claim administrators.
- Parties involved in valid legal process and have a right to receive or compel us to disclose the information.
- Government bureaus, entities, authorities and agencies, including to respond to governmental inquiries or requests or to provide information to tax authorities, regulators, supervisory authorities, the police, or courts of competent jurisdiction.
- Contractual counterparties, including patients and customers, in accordance with the requirements, expectations or responsibilities of your position, employment or services that we have contracted for you to provide.
- Potential acquirers of our business or parties with which we may enter a joint venture.
