At PM Pediatric Care our team is dedicated to providing high quality medical care to your child(ren). As a pediatric healthcare provider, we work closely with our pediatric patients and their families to ensure that the best possible medical care is delivered. This process involves close collaboration between PM Pediatric Care and your family to ensure we prioritize the patient’s best interests. Therefore, the following policy has been established regarding co-parenting.

PM Pediatric Care is committed to making payment as convenient as possible for our patients. Our policy is to collect payments via credit card, flexible spending card, cash, or check at the time of service. If you have insurance, our verification system will indicate whether you have an unmet deductible and determine your expected payment.

If you are from a Legal office or Government agency, please fax your official request to 516-207-7960. If you have any questions, please leave a detailed voicemail message for the records request department at 516-207-7960. If you are a patient/parent/legal guardian looking to obtain medical records, please click here. The records will be sent to the patient’s portal unless otherwise indicated.

This form is for parents/legal guardians who are requesting restrictions on parental rights to consent to treatment and/or access to medical records of their children. PM requires legal documentation (such as a Court Order, Restraining Order, Custody Agreement, or other legally binding order) to accompany all requests. All documentation will be reviewed prior to implementing any valid legal restrictions of those rights.
