Medical Records Request

Visit notes are automatically published to the patient portal following each visit.  Lab results are made available on the portal as soon as they are received.  Our secure PM Pediatric Care | Patient Portal gives you 24/7 access to your health information and is the fastest way to view records.

What you can do:

• View lab results, xray results, visit summaries and recent visit notes
• Download and/or print medical records
• Electronically share records (xrays) with other providers

If you are a patient or parent/legal guardian seeking to have records directed to a third party or are seeking other records that are not available on the patient portal, click on the “Start Request” button.  Please be prepared to upload any required supporting documentation, including Photo ID, necessary for verification purposes.

• All requests are processed securely.
• Identity verification is required.
• All requests are fulfilled in accordance with applicable State and Federal guidelines.  Any missing or incomplete documentation may delay processing.

If you are a parent or legal guardian seeking proxy access to the patient portal, click on the Patient Portal Proxy Access Request Form for Minor Patients   You will then be prompted to complete the “Proxy Access Request Form” and upload any required supporting documentation, including Photo ID, necessary for verification purposes.

If you are from a Legal office or Government agency, please fax your official request to 516-207-7960. A reasonable, cost-based fee may apply for printed or mailed copies.  No fee is charged to access records electronically when available.
