Patient Portal Frequently Asked Questions (FAQs)

A patient portal is a mechanism by which patients or their parents/legal guardians may access medical information. Currently, information that is available through the patient portal includes visit notes, laboratory results, and x-ray imaging.  Please note that the patient portal is different from the patient’s medical record – however, information from the patient’s medical record flows into the patient portal and pre-populates certain demographic information.  For clarity, the patient portal is separate from our online visit booking system.

Yes. The portal belongs to the patient, not the parent/legal guardian, and there is only one patient portal assigned to each patient. In other words, parents/legal guardians do not maintain their own independent account. However, for most minor children, the portal is “proxy managed” meaning that the parent/legal guardian(s) have access to log-in to the patient’s portal account.

  • Children under 12 generally do not have independent accounts without proxy access.
  • Teens 12–17 may be eligible for their own account, consistent with state privacy rules.
  • Patients 18 and older manage their own accounts.

Proxy access allows a parent or legal guardian to view a minor child’s medical information through the patient portal.

Proxy access may be granted to:

  • A parent or legal guardian of a minor child.
  • Legal guardian or durable power of attorney of an adult patient (legal documentation required).
  • For non-minor patients, the patient may authorize in writing a parent or other adult involved in their care to have proxy access.

Note: Custody arrangements may impact who is eligible for proxy access such as, for example, in cases where a parent/guardian’s legal rights have been restricted or revoked. We may request legal documents (e.g., custody orders, court-issued guardianship) to verify authority.  See our policy on Co-Parenting Expectations for more information.

Proxy access is generally established in the clinical office at the time of the patient’s first visit.

Please submit a request online to our Medical Records Department through Patient Portal Proxy Access Request Form for Minor Patients.

Yes – unless restricted by custody or other legal order.  If there is a legal restriction, we must receive documentation specifying limits to parental access.  These requests can be submitted through our website at:  Request for Review of Legal Documentation and Restriction.   All documentation will be reviewed prior to implementing any valid legal restrictions of those rights.

Please notify us immediately and provide updated court documents so that we can ensure access is accurate and compliant with legal requirements.

Yes – Each parent/legal guardian must have their own proxy portal access.  Proxy access is established by an email address/username and password.  Parents/legal guardians should not share log-in information.  Please note that proxies are responsible for complying with all usage requirements and terms and conditions of the patient portal.  Patient Portal access is provided as a convenience to patients and their authorized representatives and may be revoked at any time for any reason.

You can log in using the following methods:

  • Username/Password combination
  • Patient Name, DOB, and text authentication option. Please note that the portal only permits two phone numbers for the text-authentication option.  These two phone numbers are pulled from the patient’s medical record, and more specifically are the phone numbers listed in the “Cell” and “Home” fields in the medical record.  It is the responsibility of the parents/legal guardians to ensure that they communicate with each other and determine which phone numbers will appear in those two fields.  It is important to note that even if the “Home” phone is a cell phone number, it can be placed in this field if there are two parents needing access.
  • Additionally, if using the mobile app, you can log in via a 6-digit passcode if you choose to set one.

At age 18, proxy access automatically ends unless the patient signs an adult proxy consent form.

State and federal laws protect certain categories of adolescent health information. As a result, some notes or lab results may not be visible.

You may request medical records directly from our Medical Records department.   Please submit a request online through our website at https://pmpediatriccare.com/recordsrequest.  Please keep in mind that certain functionality may be limited in the mobile application of the patient portal, and additional features may be available when viewing the patient portal via web browser.
